Electromyography (EMG) is a diagnostic test that measures the electrical activity of your muscles to help identify and localize nerve and muscle problems behind weakness, numbness, tingling, or cramping.

What EMG is and how it works

Electromyography is a diagnostic study, not a treatment. It records the small electrical signals that muscles produce at rest and during contraction, giving your physician an objective picture of how well your nerves and muscles are communicating. When a nerve is irritated, compressed, or injured, the muscles it supplies often generate abnormal electrical patterns that an EMG can detect and help localize.

During the study, a very thin needle electrode is inserted into selected muscles. As the muscle sits relaxed and then gently contracts, the electrode picks up its electrical signals, which are displayed on a screen and played as sound. By sampling several muscles along a nerve pathway, your physician can help determine whether a problem comes from the nerve root, a peripheral nerve, the muscle itself, or the junction between nerve and muscle, and can often estimate whether an injury is recent or longstanding.

At Columbia Pain & Spine Institute, EMG is often performed together with a nerve conduction study because the two tests provide complementary information: one examines the muscle’s electrical activity, the other measures how quickly and strongly signals travel along the nerve. The combined results add objective detail about nerve and muscle function that complements your clinical exam, imaging, and — when pain localization is needed — diagnostic injections or nerve blocks, which remain the primary tools we use to pinpoint the source of pain and guide treatment.

What EMG helps evaluate

EMG is used to investigate symptoms such as numbness, tingling, muscle weakness, or cramping and to clarify nerve and muscle disorders. When pain is the main concern, diagnostic injections and nerve blocks are usually the most direct way to identify its source, and EMG can add supporting information about nerve and muscle function.

WHAT TO EXPECT

Your EMG visit, step by step

Before

No sedation is needed, so you can drive yourself and return to normal activities the same day. Bathe or shower beforehand and avoid lotions, oils, or creams on your skin that day. Tell your physician about any blood thinners you take or if you have a pacemaker or other implanted device. Wear loose, comfortable clothing that allows access to the areas being studied.

During

EMG does not use image guidance; your physician selects muscles based on your symptoms and exam. A fine needle electrode is placed into each muscle, and you will be asked to relax and then gently tense it. You may feel a brief pinch or pressure as the needle moves. Most studies take about 30 to 60 minutes, depending on how many muscles and nerves are examined.

After

There is essentially no recovery period. You may notice mild soreness or a small bruise at the needle sites for a day or two, which usually resolves on its own. You can resume work, driving, and regular activities right away. Your physician interprets the results and discusses next steps, which may include further imaging, injections, or a referral.

Common questions about EMG

Does an EMG hurt?

Most patients tolerate EMG well. You may feel a brief pinch or a deep ache when the needle electrode is inserted and when the muscle contracts, but the discomfort is short-lived and no anesthesia is required. Any soreness afterward is usually mild.

How is EMG different from a nerve conduction study?

They measure different things and are frequently done in the same appointment. A nerve conduction study uses surface electrodes and small electrical pulses to measure how quickly and strongly signals travel along a nerve, while EMG uses a needle electrode to record the muscle’s own electrical activity. Together they give your physician a fuller picture of nerve and muscle function.

What happens after my EMG results come back?

Because EMG is diagnostic, the findings help guide your treatment plan. Depending on the cause, your physician may recommend a targeted procedure such as a carpal tunnel injection, an epidural steroid injection for nerve root irritation, or another therapy tailored to your diagnosis at our Gresham or Portland, Oregon clinic.
