How to refer a patient
To refer a patient, please fax the patient’s records to our office. Our team will review the information and coordinate the next steps in their care.
What to include
To help us review each referral promptly, please send the following with the patient’s records:
- Relevant clinical notes and history
- Imaging studies and associated reports
- Insurance information
- Patient contact information
Common reasons for referral
