Platelet-Rich Plasma (PRP) Therapy

Platelet-rich plasma (PRP) therapy is a regenerative injection that concentrates the platelets from a small sample of your own blood and delivers them, under image guidance, into an injured joint, tendon, or other soft tissue.

OVERVIEW

What PRP therapy is and how it works

Your blood contains platelets, cell fragments rich in the growth factors your body naturally uses to repair tissue. PRP therapy starts with a small blood draw, which is spun in a centrifuge to separate and concentrate those platelets. The concentrated plasma is then injected directly into the area of injury, with the aim of delivering a higher local dose of growth factors than the body would route there on its own to support the healing process.

Because PRP is prepared from your own blood (an autologous treatment), there is no risk of rejection or of an allergic reaction to a foreign substance. At Columbia Pain & Spine Institute, our fellowship-trained PM&R physicians use ultrasound or fluoroscopic guidance to place the injection precisely within the targeted tendon, ligament, or joint.

An emerging therapy. PRP is a developing area of regenerative medicine. Research is ongoing, the level of evidence differs from one condition to the next, results vary from person to person, and PRP is not a guaranteed cure. Your physician will discuss whether it is a reasonable option for your specific diagnosis and how it fits alongside other treatments.

PRP is most often considered for chronic tendon, ligament, and joint problems that have not responded well to rest, activity changes, physical therapy, or other conservative care. PRP is sometimes considered for osteoarthritis or tendon problems in other joints, such as the hip or shoulder, though the supporting evidence there is more limited. Your physician will review your imaging and history to judge whether your particular injury is a good candidate.

WHAT TO EXPECT

Your PRP procedure, step by step

Before

Tell your physician about all medications and supplements you take. Because PRP relies on your body’s inflammatory healing response, you may be asked to pause anti-inflammatory medications (NSAIDs) for a period before and after the procedure. The visit begins with a small blood draw from your arm, much like a routine lab test.

During

Your blood is spun in a centrifuge for several minutes to concentrate the platelets. The injection site is cleaned and numbed, and your physician uses ultrasound or fluoroscopic guidance to deliver the PRP precisely into the injured tissue. The full appointment typically takes about an hour, and the injection itself only a few minutes.

After and recovery

Mild soreness, swelling, or aching at the injection site for a few days is common and is part of the expected healing response; ice and acetaminophen are often suggested rather than anti-inflammatories. Most people return to normal daily activity quickly but are advised to ease back into strenuous exercise. Because PRP works gradually, any benefit tends to develop over weeks rather than immediately, and some conditions may call for a series of injections. Your physician will give you a recovery and follow-up plan tailored to your injury.

Therapeutic, not diagnostic. PRP is a therapeutic injection intended to support tissue healing, not a diagnostic test. If the source of your pain is still unclear, your physician may first recommend imaging or a diagnostic procedure to confirm the target before treatment.

Is PRP made from my own blood?

Yes. PRP is autologous, meaning it is prepared entirely from a small sample of your own blood drawn at the time of your visit. Nothing from a donor or any synthetic material is added, which is why there is no risk of rejection or of an allergic reaction to a foreign substance.

How long until I notice a difference?

PRP supports a gradual healing process, so any improvement typically develops over several weeks rather than right away.
