These terms govern the text messages Columbia Pain & Spine Institute sends to patients about their care — what we send, how often, and how to stop at any time.

By opting in to receive SMS messages from Columbia Pain And Spine Institute (“we,” “us,” “our”), you agree to these Terms and Conditions (“Terms”).
SMS Messaging Service
By providing your mobile number and opting in to receive text messages from Columbia Pain And Spine Institute, you agree to receive customer care messages related to your interactions with our practice — appointment reminders, appointment confirmations and scheduling, care coordination, billing and account notifications, and replies to questions you send us. Message and data rates may apply.
How You Opt In
We send text messages only to patients who have given us their mobile number and agreed to receive them. You can give consent by:
- Completing the text message sign-up form on this website
- Checking the text messaging box on our patient intake or update paperwork
- Enrolling through our patient portal
- Telling a member of our staff by phone, which we record in your record
Message Frequency
Message frequency may vary. On average, 1–2 messages per month. You will get more than one message from us unless you opt out. Columbia Pain And Spine Institute reserves the right to alter the frequency of messages at any time to increase or decrease the total number of messages. Columbia Pain And Spine Institute and carriers are not liable for delays or undelivered messages.
Message and Data Rates
Message and data rates may apply depending on your mobile carrier and plan. We do not charge you for the messages we send; any charges come from your own mobile plan.
Privacy Policy
Your information will be handled in accordance with our Privacy Policy. We do not share your personal information, phone number, or SMS consent opt-in data with third parties or affiliates for marketing or promotional purposes.
Consumer data, including phone numbers and SMS consent data, will not be transferred, shared, disclosed, or provided to any external organization. The only exception is where such disclosure is strictly necessary to deliver the SMS service on the company’s behalf and is permitted by applicable rules.
Cancellation / Opt-Out Instructions
You may opt out of receiving text messages at any time by replying STOP to any message. For assistance, reply HELP. After you opt out of text messaging, you will receive one additional message confirming your request has been processed, and you will not receive further text messages from us.

If you want to rejoin later, you can sign up again at any time using the sign-up form or by asking our staff.

Supported Carriers
Our text messaging service is supported on major U.S. mobile carriers. Carriers are not liable for delayed or undelivered messages.
Eligibility
You must be at least 18 years old, and the account holder or authorized user of the mobile number you provide, to opt in. Where we treat a minor patient, the parent or guardian provides consent and receives the messages.
Liability
We are not responsible for any charges, errors, or delays in SMS delivery or undelivered messages caused by your carrier or third-party service providers.
Changes to These Terms
We may update these Terms from time to time. The Effective Date at the top of this page shows when they were last revised, and the current version will always be posted here.
