This is not our HIPAA Notice of Privacy Practices. Protected health information created or received in the course of your treatment is governed separately by our Notice of Privacy Practices, which you receive as a patient of the practice and may request at any time by calling (503) 382-8100.

We collect your information directly from you — for example when you complete a form on this website, complete paperwork at one of our offices, or speak with our staff by phone — and automatically, such as when you interact with this website.

  • Coordinate your care and respond to questions you send us

Text Messaging (SMS)

What we send

Our text messaging program is used only for communication related to your care with us: appointment reminders, appointment confirmations and scheduling, care coordination, billing and account notifications, and replies to questions you send us.

We do not send marketing or promotional text messages.

How you opt in

We text you only after you have given us your mobile number and agreed to receive text messages — on a form on this website, on paperwork at one of our offices, through our patient portal, or verbally with a member of our staff, in which case we record your consent in your record. Consent to receive text messages is never a condition of receiving treatment from us.

Message frequency and rates

Message frequency may vary. On average, 1–2 messages per month. Message and data rates may apply depending on your mobile carrier and plan.

How to stop

You can opt out of receiving SMS messages at any time by replying STOP to any message we send you. After you opt out, you will receive one additional message confirming your request has been processed. You may also ask any member of our staff to remove your number. We do not share your personal information, phone number, or SMS consent opt-in data with third parties or affiliates for marketing or promotional purposes. Mobile information collected for text messaging is not sold, rented, or shared for marketing, and is not shared with any third party except the messaging service provider that delivers the messages on our behalf.

Data Sharing and Transfer

Consumer data, including phone numbers and SMS consent data, will not be transferred, shared, disclosed, or provided to any external organization. The only exception is where such disclosure is strictly necessary to deliver the SMS service on the company’s behalf and is permitted by applicable rules.

We do not sell or rent your information, and we do not share your personal information, phone number, or SMS consent opt-in data with third parties or affiliates for marketing or promotional purposes.

The only party covered by the exception above is the telecommunications provider that transmits our text messages on our behalf. It is permitted to use your information solely to deliver those messages for us, and is bound by contract to protect it.

Protection of Information

We take steps to protect your information against unauthorized use or disclosure. No method of transmission or storage is completely secure, however, and we cannot guarantee absolute security. Please do not send sensitive medical details by text message or email; call us at (503) 382-8100 instead.

Your Choices and Controls

  • Reply STOP to any text message to stop receiving them
  • Ask us to correct or update the contact information we hold for you
  • Ask us what contact information we hold for you
  • Ask us to remove your contact information, subject to the records we are required to retain by law

Children’s Privacy

This website is not directed to children under 13, and we do not knowingly collect information from them through it. Where we treat a minor patient, we communicate with the parent or guardian who has provided consent.
